Coordinated Service Planning (CSP) – For children 0-18 years or 19-21 years (attending school)
We support children and youth with multiple or complex special needs by offering a seamless, family-centered service. A Service Plan Coordinator works with families and service providers to create a Single Plan of Care that is tailored to the child or youth’s specific needs and the services the family is receiving. This approach ensure that all local providers are working collaboratively towards the same goals.
Referrals can be made when a child or youth’s needs are identified as multiple or complex special needs.
Fetal Alcohol Spectrum Disorder Program– For children 0-18 years or 19-21 years (attending school)
This program offers support to children, youth and families impacted by Fetal Alcohol Spectrum Disorder (FASD). We provide helpful information, guidance, assistance in finding services, planning for the future, and connecting families with others who understand what you’re going through.
Assessment and Consultation-For children 6-17 years
Children and youth already receiving our services may be eligible for a Psychological or Psychiatric assessment. Assessments help identify a diagnosis and guide treatment if needed.
Developmental Services Programs– For children 6-17 years
We provide services to children, youth, and their families with a developmental disability and a behavioral or mental health concern.
Services include individual, parent mediated services to help support your child’s development, and mental health needs.
Autism and Related Disorders Program – For children 6-17 years
We help children and youth with autism spectrum disorder and their families by offering support to encourage their growth and development.
Services include individual, parent mediated services to help support your child’s development, and mental health needs.
Ontario Autism Program (OAP) Urgent Response Services (URS)
We offer short-term, rapid-response support to help reduce the risk of harm to your child, others, and/or property. Families, registered with the Ontario Autism Program, who qualify can receive up to 12 weeks of services tailored to address a specific behaviour of concern.
Our URS Coordinator can connect your with helpful expertise to support with strategies for managing your child’s behaviors. We work with both families and professionals to make sure everyone has what is needed to help your child or youth.
We provide support services in the following communities: Bruce-Grey, Chatham-Kent, London-Middlesex, Huron-Perth, Oxford-Elgin, Sarnia-Lambton and Windsor-Essex.
